- Search for patient:
- If patient is active and exists, check in with Joel for now
- If patient does not exist, Add Patient under following
- Account – Michigan – Oakland County – Pretrial Services
- Program – Oakland County – Pretrial Services
- Upload the referral form. Make sure the following information is added.
- Name
- DOB
- Gender – Guess your best
- Ethnicity – Unknown
- PID2 – Docket Number
- Frequency – Set as Manual to start
- Panel (Use Custom Panel if not in Panel Group)
- Case Manager (PT Supervision)
- Associated Account – Court listed on Referral
- Direct Email Distribution (ptsupervision@oakgov.com)
- Collection Notes – Start Date and Testing Information
- Set Collection Location
- Attach Referral to Paper Clip
- Input information into JAMS spreadsheet
- Employee Name
- Patient Name
- Program Name
- Collection Site – JAMS or ADAM
- Start Date
- Attended
- BAC – If necessary
- Referral Attached
- Frequency
- Frequency Updated
- Negative Results sent
- Ticket Number
- Check each day until client arrives or start date
- If patient does not arrive by start date, email ptsupervision@oakgov.com and put in a new start date if they provide one. If they don’t give a new start date, keep checking until start date is 2 weeks old. If no attendance by 2 weeks, remove from spreadsheet
- If patient does arrive, update Frequency group to the highest frequency listed on the referral form (should be listed on the spreadsheet).
- Check Test History daily until results report.
- If results are negative, email them to ptsupervision@oakgov.com
- If results are positive, nothing left to do
- Once complete, remove the client from the JAMS spreadsheet
