Michigan PTS Referral Notes

JAMS

  1. Search for patient:
    1. If patient is active and exists, check in with Joel for now
    2. If patient does not exist, Add Patient under following
      1. Account – Michigan – Oakland County – Pretrial Services
      2. Program – Oakland County – Pretrial Services
  2. Upload the referral form.  Make sure the following information is added.
    1. Name
    2. DOB
    3. Gender – Guess your best
    4. Ethnicity – Unknown
    5. PID2 – Docket Number
    6. Frequency – Set as Manual to start
    7. Panel (Use Custom Panel if not in Panel Group)
    8. Case Manager (PT Supervision)
    9. Associated Account – Court listed on Referral
    10. Direct Email Distribution (ptsupervision@oakgov.com)
    11. Collection Notes – Start Date and Testing Information
    12. Set Collection Location
    13. Attach Referral to Paper Clip
  3. Input information into JAMS spreadsheet
    1. Employee Name
    2. Patient Name
    3. Program Name
    4. Collection Site – JAMS or ADAM
    5. Start Date
    6. Attended
    7. BAC – If necessary
    8. Referral Attached
    9. Frequency
    10. Frequency Updated
    11. Negative Results sent
    12. Ticket Number
  4. Check each day until client arrives or start date
    1. If patient does not arrive by start date, email ptsupervision@oakgov.com and put in a new start date if they provide one.  If they don’t give a new start date, keep checking until start date is 2 weeks old.  If no attendance by 2 weeks, remove from spreadsheet
    2. If patient does arrive, update Frequency group to the highest frequency listed on the referral form (should be listed on the spreadsheet).
      1. Check Test History daily until results report.  
      2. If results are negative, email them to ptsupervision@oakgov.com
      3. If results are positive, nothing left to do
  5. Once complete, remove the client from the JAMS spreadsheet
