If ER (Emergency Referral):
- Mark Yes Active
- Group stays on manual, phase = NA.
- Schedule Manual Order via testing page: Use BOTH Std rapid + Std Panel.as panels
If Enrollment:
- Mark No on active.
- Assign Phase 1 (6–8 times/month).
- Add notes on summary page about enrollment referral.
- If referral requests manual spot test instead of RDT, replace accordingly.
Additional Notes
- Any panel that is different from a standard panel must be approved by Saundra Muro Jessica Carrillo, Jose Vargas, Frances Castaneda, Roseann Nelson, & Amy Yang.
  - Put notes of who approved in Review notes and your initials, ticket number and date
- Fee Waiver – If you get a fee waiver create a Voucher. Sent the Voucher as Good Until and set the date they list. Put the ticket number in the voucher note.

- Frequency Group:
  - 6-8 times per month is phase 1
  - 4-5 times per month is phase 2
  - 3-4 times per month is phase 3
  - 1-2 times per month is phase 4
