1. Most frequent type(s) of requests this customer has? Fresno County DSS wants to add a fee waiver to the account and or test a patient the same day with a rapid test result  

2. Are any programs integrated with a Case Management System? (Y/N).  

3. What are some examples of “Today” requests a customer might ask? If the patient is enrolled. If the patient has a fee waiver on file. 

4. Do customers issue any vouchers? (Y/N) Would this be a “Today” request? Yes 

5. Does this customer submit manual referrals? Do these samples come in via email ahead of time at time of collection, or does the client walk in with the form.  If so, please upload example into your respective state folder Region 1- CS Centralization. 

. 6. What methods of payment are accepted if a client has a co-pay? Cash or card minimum card payment we accept is $10.00 and .50 cent for the card transaction a total of $10.50 

7. Do you accept Private Pay clients? (Y/N) If so who is eligible? Yes we do anyone who is not in a current program. 

8. Can we add new users to Aversys for all accounts? That is up to who is ever in charge of the account at the time. 

9. Do we refund for negative confirmations or any testing outcomes? No
